A ‘Notice of Intended Marriage’ (Form 13) MUST be submitted to me at least one month and one day prior to the date of your intended marriage. This form can be lodged with myself up to 18 months prior to the date of your marriage. This document can be witnessed by an authorized celebrant or in the presence of a Justice of the Peace, Commissioner for Declarations, barrister, solicitor, a legally qualified medical practitioner or by a member of the Australian Federal Police or the police force of a State or Territory. Form 13 is available from myself or download from www.ag.gov.au/celebrants.

BIRTH CERTIFICATES/PASSPORTS - Original birth certificates are required to be produced if parties are born in Australia. If parties are born overseas and birth certificates are not available, please produce your overseas passport. Please discuss any difficulties regarding the production of these documents with myself. Birth certificates are obtainable from the Registrar of Births, Deaths and Marriages of the capital city in the State where born.

DIVORCE CERTIFICATE/DECREE NISIIf either party has previously been married, evidence of the dissolution of that marriage MUST be produced prior to the date of your marriage. These certificates are obtainable from the Federal Family Court.

DEATH CERTIFICATE If the party intending to be married is a widow/widower from a previous marriage, a original ‘Death Certificate’ must be produced. This certificate is obtainable from the Registrar of Births, Deaths and Marriages in the capital city of the State where death occurred.

CERTIFICATE OF MARRIAGE - An official ‘Certificate of Marriage’ will be provided by myself on the wedding day. This will be signed by the Bride and groom, two witnesses of 18 years of age or over and by myself. Some government departments or banking institutions may require a certified copy of the certificate of marriage which is obtainable from the office of the Registrar of Births, Deaths and Marriages. The payment of a fee will be required by that government office re the supply of this certified document. I can assist you with this application if you require a certified copy of this certificate.

PLEASE NOTE THAT THE CEREMONY CANNOT PROCEED UNLESS ALL REQUIRED DOCUMENTATION HAS BEEN PRODUCED.
